I figured out what the issue is.  I have both VS2008 and VS2005 on my machine.  When I opened my VS2005 the object mapper was there.  When I opened my VS2008 the object mapper was missing.

The documentation lists IdeaBlade.DevTools.Orm.AddIn as the file to rename.  You have to rename BOTH the dll and the addin  files in that directory, not just 1.  If you have VS2008 then you have to rename the IdeaBlade.DevTools.Orm.VS2008.AddIn file as well.
 
It's pretty obvious but I followed the documentation to the letter with no interpretation and that's why it didn't work for me the first time.

The Addin is in a different location for Windows 7. Try:
C:\ProgramData\Microsoft\MSEnvShared\Addins

As noted above I've tried the steps above and it didn't work.  The Troubleshooting guide then says to contact support.

Hi there,
 
I'm still using 3.6.6.1 and haven't had the time to upgrade to the latest version.  Has anyone seen any issues with the object mapper not showing up in visual studio 2008 running on windows 7?
 
I'm pretty sure when I had VS2005 installed the object mapper was there.  Then when I opened VS2008 the object mapper was missing. 

I performed the steps in the installer manual on troubleshooting this issue and they didn't work.
